Chelsea:1 Hazard 45′ Crystal Palace: 0

Chelsea secured their fourth Premier League title as Eden Hazard’s penalty rebound defeated Crystal Palace.
PFA Player of the Year Hazard followed up to head in after his weak spot-kick was saved by keeper Julian Speroni.
The Belgium forward won the penalty just before half-time after tumbling over James McArthur’s challenge.
Jason Puncheon and Wilfried Zaha went close in the second half for Palace, but Chelsea had chances too as they earned a fifth English title overall.
